A significant expansion in access to treatment helped slash the number of AIDS-related deaths in 2010, bringing the number of people living with HIV to a record 34 million, the United Nations said Monday. “We are on the verge of a significant breakthrough in the AIDS response,” said Michel Sidibe, executive director of UNAIDS. “New HIV infections continue to fall and more people than ever are starting treatment,” he noted.

Speaking to journalists in Berlin for the presentation of the report, Sidibe hailed what he called a “game-changing year.” “For the first time we are able to demonstrate that if you put people early on treatment you can reduce the number of new infections,” he said.

About half of those eligible for treatment are now receiving it, with the most dramatic improvement in access seen in sub-Saharan Africa, which recorded a 20-percent jump in people undergoing treatment between 2009 and 2010. As a result of better access to healthcare, the number of AIDS-related deaths was also falling, said UNAIDS, the UN agency spearheading the international campaign against the disease.

In 2010, 1.8 million deaths were linked to AIDS, down from a peak of 2.2 million last seen in 2006. “An estimated 700,000 AIDS-related deaths were estimated to have been averted in 2010 alone,” added the UN agency. Not only is treatment helping to prevent new AIDS-related deaths but it is also contributing to a drop in new HIV infections.

Patients undergoing care were less likely to infect others, as prevention programmes coupled with treatments were proving effective. Modelling data suggests that “the number of new HIV infections is 30 to 50 percent lower now than it would have been in the absence of universal access to treatment for eligible people living with HIV.” In Namibia for instance, where treatment access reached an all-time high of 90 percent and condom use rose to 75 percent among men, the combined impact contributed to a 60 percent drop in new infections by 2010, noted UNAIDS.

“I want to say this report is clearly showing that even in those difficult period (three years of financial crisis) we are still having a result: we have more and more countries which are reducing the number of new infections,” said Sidibe. The UN agency added that the full preventive impact of treatment was likely to be seen in the next five years, as more countries reach high levels of treatment coverage.

“The massive increases in the numbers of people receiving treatment in South Africa between 2009 and 2010, for example, are likely to be reflected in substantially fewer new infections in the near future,” it said. UNAIDS assessed that even if the AIDS epidemic is not over, “the end may be in sight if countries invest smartly.”

“In the next five years, smart investments can propel the AIDS response towards achieving the vision of zero new HIV infections, zero discrimination and zero AIDS-related deaths,” it said. However, the turning point is coming at a time when industrialised nations’ public budgets are being squeezed and translating to less international funding for the AIDS response.

Medical charity Medecins Sans Frontieres urged governments to keep up their funding. “Never, in more than a decade of treating people living with HIV/AIDS, have we been at such a promising moment to really turn this epidemic around,” said Tido von Schoen-Angerer, who heads MSF’s access campaign.

“Governments in some of the hardest hit countries want to act on the science, seize this moment and reverse the AIDS epidemic. But this means nothing if there’s no money to make it happen.”

Derma Sciences Inc expects to find a deep-pocketed partner for its treatment for diabetes-induced foot ulcers in the first half of 2012, even before it enrolls patients for late-stage trials. Derma Sciences specializes in products that treat wounds from common burns to deep skin gashes, with annual revenue of $56.5 million.

Its newest product, a topical gel dubbed DSC127, could reap over $1 billion in worldwide annual sales if approved, according to its chief executive. “We are fielding inquiries from a number of multinational pharmaceutical companies about DSC127,” CEO Edward Quilty told Reuters in an interview, without providing any details. “There is a high likelihood that the partnering will happen before we enroll the first patient in the Phase 3 trial.”

In May, the company reported additional positive data from a mid-stage trial testing the drug in 80 patients with diabetic foot ulcer (DFU). It plans to start two late-stage trials that will cost about $30 million and run parallel to each other in the first half of next year, Quilty said.

“We have enough money to complete these trials on our own,” he said, referring to new capital raised in June. A partner would help the company market the drug in the United States and overseas, beyond the capabilities of its own staff. Derma Sciences, based in Princeton, New Jersey, plans to increase its salesforce to 80 from 25 by 2015. It sells its existing wound-care products — Medihoney, Bioguard, Xtrasorb and Algicell — in hospitals, nursing homes and other health care facilities.

“Our salesforce could reach around 1,200 wound clinics in the U.S., but we feel the salesforce is not big enough to cover all the primary care physicians and podiatrists,” Quilty said. Companies with large sales teams and a strong presence in wound healing include Kinetic Concepts Inc, Smith & Nephew and Shire.

Quilty, who has more than 35 years of experience in the healthcare industry, joined Derma Sciences from biopharmaceutical company Palatin Technologies in 1996. U.S.-based LBI Group Inc, Jennison Associates LLC and New Zealand-based Comvita, which supplies medical grade honey for Medihoney, are the three largest shareholders of the company. Shares in the company jumped 25 percent when it released its latest data on DSC127 on May 25 to $11.65, but have since retreated to under $8 with a broader market sell-off. Analysts see the stock hitting $19-$21 in the next 12 months.

AVERTING AMPUTATION

DSC127 works by increasing the number of mesenchymal stem cells (MSCs) at the site of injury. MSCs have the ability to differentiate into various types of cells found within the human body, including muscle cells, bone cells, and skin cells.

The most recent data shows that at the end of 24 weeks, foot ulcers in 73 percent of patients treated with DSC127 healed completely, compared with 46 percent of patients treated with a placebo. There were no drug-related adverse safety events during the study period.

About 3 million diabetic patients develop foot ulcers in the U.S. annually, and about 14 percent to 24 percent of them require amputation.

Quilty, who expects peak annual DSC127 sales of over $300 million in the U.S. and over $1 billion worldwide, said the high healing rates with better safety profile will differentiate it against current drugs for diabetic ulcers like Healthpoint Biotherapeutics’ Regranex, Shire’s Dermagraft and Organogenesis’ Apligraf. Dermagraft and Apligraf are both bioengineered tissue products that contain living skin cells to help healing, while Regranex is a topical ointment.

Derma Sciences expects its advanced wound care sales to grow in the coming years.

“We reported strong sales in the second quarter and we expect to report solid third-quarter sales,” said Quilty.

For the second quarter, Derma Sciences reported better-than-expected sales of $15.9 million, helped by a 60 percent jump in sales of its flagship product Medihoney. It is a dressing for the management of chronic wounds and burns that contains active leptospermum honey, produced from bushes that grow in Australia and New Zealand.

A new analysis of earlier research finds that both higher vitamin D intake and higher blood levels of the vitamin’s active form are linked to lower risk of colon and rectal cancers. In 18 studies that included more than 10,000 people, colon cancer risk was as much as 33 percent lower in subjects with the highest blood levels of vitamin D compared to those with the lowest levels, researchers report in the Journal of Clinical Oncology. Those with the highest intake of vitamin D through supplements and food had 12 percent lower risk than those with the lowest intakes.

The total of studies available for analysis is still sparse, noted senior author Dr. Huanlong Qin and his colleagues from The Sixth People’s Hospital affiliated with Shanghai Jiao Tong University in Shanghai. Additional studies would be “highly desirable to enable more precise estimates and a better understanding of the role of vitamin D in colorectal cancer carcinogenesis,” they write.

Vitamin D has previously been linked to protection from various cancers, heart disease, diabetes and asthma, among other conditions. How the vitamin might exert a beneficial effect is still poorly understood, however. Some evidence suggests that to achieve a benefit people may need more than current recommended daily requirements.

Up to 58 percent of U.S. adults and adolescents may have vitamin D deficiency, which is “an important health problem in the industrial world,” Qin’s team said. While there are biologically plausible mechanisms through which low vitamin D levels could increase colon cancer risk, the researchers add, studies have had mixed results so far.

A recent review by the U.S. Institute of Medicine concluded there isn’t enough information to justify increasing recommended intakes of vitamin D.

For their analysis, Qin’s group looked only at the strongest studies that investigated the relationship between vitamin D intake or blood levels of 25-hydroxyvitamin D, the active form in the body, and colorectal cancer, colon cancer or rectal cancer.

They identified nine studies of vitamin D intake including 6,466 patients, four of which were conducted in the US, three in Europe, and two in Asia. Nine more studies, including 2,767 cases and 3,948 people in comparison groups, looked at blood levels of 25-hydroxyvitamin D, with six conducted in the US, two in Europe and one in the US. One study included both vitamin D intake and blood levels of 25-hydroxyvitamin D.

Limitations of the analysis, the researchers note, included the lack of uniform criteria for comparison groups across the various studies; also, not all of the studies had information on vitamin D intake or its blood levels for individuals, only ranges for groups. The “lowest” and “highest” categories in different studies also varied significantly. Hence, the researchers’ conclusion that their findings need to be confirmed in large, gold-standard randomized clinical trials of vitamin D supplements.
